generally speaking, you are as safe on the placebo week as you are on the other 3 weeks. They do make a 31 day pill that delivers hormones in a low dosage for the entire time. You do not have a period.
he doesn't have to *** all inside of her, it can be pre ***. a little drop, and i doubt it would happen if she was on her pills that turn into her period cycle.

Even if you don't miss any pills, there's a small chance that you could become pregnant. That happened to a friend of mine. That "small chance" is now 35 :-)
It is always possible, those pills are not 100% foulproof. However, the odds are no greater during the sugar pill week than during any other: birth control prevents the ovaries from releasing any eggs by convincing your body that you are pregnant, so there can not be any egg in the uteris unless the pills were not taken properly or failed to work (which is rare unless the person taking them did so incorrectly).
YUP. Pre ejaculation and you are ALWAYS able to become pregnant when having sex..unless you are already pregnant.
