I don't know about medicaid but breast reduction is a recognized method to help reduce severe back pain. You might have to be real persistent to get it done but keep at them.
My personal observation is a lot of insurers will always say no banking on people accepting that for an answer but the ones that keep after them have a higher chance of getting the help you need.

You should call Medicaid and ask them about coverage. Then go to a surgeon for a consultation and they can tell you, based on things like height and weight and amount of pain, if you're eligible. They send pictures and documentation to your insurance company in order to get you approved.
