I would have to ask, do people who get their hair styled, nails done, put on make-up and generally care about their appearance have poor self-esteem? Possibly? When someone is doing something for themselves that makes them feel good, it could be on one hand because they have high self-esteem, or it could be because they don't feel good enough inside. A great way to guess about someone's self-esteem is to see how they treat themselves and other people. That is the most obvious way to ascertain one's self-esteem.

In my opinion, I think you posted this question in an attempt to let the world know that you are better than someone who has had plastic surgery. Thus you maybe suffering from self-esteem issues.
And yes boobs grow bigger as you gain weight. Breast tissue is mostly fat. And someone who is really fit and have low body fat % probably have very small breasts and will consider implants (used to work in the fitness industry).
You should go to every single person who has had plastic surgery and give them a self esteem questionnaire. Then you will have the answer.
You have really answered your own question here. Yes people who have plastic surgery do have self-esteem issues why else would they be having plastic surgery in the first place?
