I think it's genetic. If your mama was born with a big waist chances are you will probably have one too and vice versa.

If you're thin and fit already, but still don't have a skinny waist, you have what your parents gave you, and even surgery will have only a small effect.
If you do regular exercise, then make sure you are working your obliques as well as your abdominals, as that can help tighten your waistline a little more.
But if you aren't in great shape, then you've got to lose the extra fat, and build muscle. To do that, there's no other way but exercise and diet. Work up to 4-5 times a week, for 30 minutes a day, and watch what you eat.
Keep a food diary. When you start, don't worry about counting fat or calories, just notice how much and what you are eating. Know what foods are good and bad for you. Just seeing it written down can be a surprise - you don't always notice how much you really eat when you eat it, but you sure notice when your jeans don't fit anymore. Watching what you eat helps you stay focused and eat healthier.

A waist is usually your bone and how your skin and body fat cover it. Due to genes there may be a small variety in how the bone is set, yet you can usually have a small waist by exercising and eating way less junky foods and replacing them with fruit. Drinking 8 glasses of water a day helps as well..pretty much eat healthy and stay in shape is the way to go. (Maybe not the easiest but a way none the less).

Unfortunately, genetics have an awful lot to do with how our bodies are shaped. Does your mother have a thick waist? You can usually judge how you're going to turn out by looking at her. Although, my Mom is pretty thin and I definitely ended up with Dad's ribcage structure. Sadly, I have resorted to liposuction. It's really the only way to sculpture your figure besides big-time attendance at the gym! I'm not saying that lipo should be done in lieu of exercise. Just that if you've tried working out and eating healthy (as I have!), lipo may be the only way to truly sculpt the body you're looking for.
