In an xray can the appendix and overy be mistaken for eachother?

Normally X-rays do not do well on soft tissue. an MRI is what would show soft tissue much better.

Simple answer - NO. If they are, change doctor!
You can't see an ovary or an appendix on xray, they do a very poor job of showing soft tissue. But to answer your question they are not in the same area of the body, so not likely.
