In a routine medical it is to make sure that you are fit for the intended sport.
For example an uncontrolled epileptic who wishes to swim (an extreme example).
It is possible that you have an unknown heart murmur which could be found on routine medical.
Now regarding your pee. This would be tested for protein and sugar.
Sugar may indicate undiagnosed diabetes
Protein could indicate undiagnosed kidney disease

No, not normally. I never had to pee in a cup for physicals. The only time I have to is for my GYN physicals.

Depends at what level you're competing. And no matter what...DON'T pee in a large container marked GATORADE. They won't be able to tell the difference!!
Yes, you can to give a urine sample but it is done in private and you just leave the cup in a container in the bathroom.
No...not usually or atleast when i had sport physicals I never needed to...but it really depends on what level you are playing. I never had to when I was getting sports physicals for high school but if you are competing at higher levels than high school you might need to.
