No, not every one bleeds even if they bleed it will be like spotting and not a whole flow.

I can only tell you from personal experience that I didn't bleed at all. I suppose some people might bleed a little more, but it's not a lot.

Depends on the person. The hymen may already be broken from using a tampon, etc. I know from experience that I did not, but was very sore.

Depends on a number of factors like how it happens, how rough it happens, how old the girl is, and just individual factors. Some people don't bleed at all, most seem to bleed a little and some girls actually have important veins attached to their hymens and they bleed out and need to see doctors (extremely rare).
