Breast reconstruction due to excessively sized breasts causing physical pain IS usually covered. However, if you want to simply restructure them (because you don't like their shape), or you want implants then the answer is no. Medicines for the procedure however usually are (such as antibiotics & pain relievers). The only exception to this is if you're having breast augmentation due to a medicinally necessary mastectomy (for breast cancer, for example).

Usually only if your policy specifically covers cosmetic surgery, OR if your mental health counselor states clearly that it is necessary for your mental health.

If you can prove that it is effecting your back & causing you back pain, I think it might be. It would make sense if it were. But I think that's the only way it might be covered b/c that is a cosmetic surgery & those usually aren't covered unless it is effecting your health somehow.
I would think if it's causing some other sort of medical problem such as constant rashes ect. that maybe but it would have to be your doctor to recommend it. For looks only, I doubt it.
