Because not everyone does. Your hymen may have spread apart during intercourse, you may have broken your hymen beforehand, or you may just not have a hymen.

The bleeding is usually from the breaking of the hymen and the stretching of the virgin vaginal walls. But if you've been intimate in other ways (fingering, toys) then your hymen may have already been broken and your walls easily relax during penetration. Less friction, less bleeding.
it really all depends. you bleed when you first do it because it breaks a tissuey band that is around your vaginal opening. most people break their hymen from horseback riding, tampons, fingering, toys, stuff like that. so, if you have done any of the above, chances are your hymen has already broke.
