I have always wondered that myself. I suspect whatever it was that the 1500's were very smelly.
I believe they used rags and old pieces of cloth. Hence the term 'ragging'.

Rags, mainly. And sea sponges for menstrual use have been around since ancient times. You can still buy them today.
although she's not from the 1500's, my 88 yo grandmother said "now you know where the term "on the rag" came from"..
