If you're 12 it shouldn't be that big of an issue. However if you're that self concious about it, just tell your mom that it bothers you and you want it done. Make sure she know's that it's not for vanity reasons, she should feel that it hinders your social life.
Precisely that you need your upper lip and eyebrows waxed, shes a girl too so hopefully she will understand.

At 12 I don't understand why you need to get your upper lip and eyebrows waxed. If you don't like the way they look then use an eyebrow shaper and do it that way. As for the upper lip you can bleach that easily.

Just ask her, if she says no just pluck it. But you should just leave your eyebrows as they are... I don't like how mine look so I just pluck out the hairs that bother me.

Tweeze your eyebrows and use either Nair or tweeze your upper lip. That's what other teens do.
Nobody gets their eyebrows and upper lip waxed at 12 years old. Wait until you're older (15 or 16), then ask her.
I am 12, so this is from my point of view.
I would suggest asking her when the first time she got hers done, and then asking her If you could-but if she says no, be prepared. Tell her that this is something you really want to do, and explain that this is common among your friends.

If you feel that you want to get rid of the hair, you should just tell your mom. Tell her that it makes you feel really self concious because you think you have a mustache. She will probably allow you to get you eyebrows waxed. But when you wax hair, it usually grows back darker so you'll have to go back.

I would just ask what is the worst that is going to happen she might say no...Personally if you were my child if your eyebrows were bad I would definitely do those for you but i would not allow my daughter to get her lip done...You are too young and the soft fuzz on your face an lips is going to change over the years...If you start waxing now you could stimulate growth of additional hair follicles causing even worse growth when if you just leave it alone it may go away over time...You also may have better luck if you just ask for your eyebrows...
