If you refuse to give an answer, then they won't be able to help you, so you may as well cancel the appointment now. Seriously, grow up - doctors deal with this kind of thing on a daily basis, and while it might be a little embarrassing to talk about, there are reasons why they ask these questions, it's not just for their own interest. I assume your periods are irregular, which is why you're being checked for a hormonal imbalance, but if you won't tell the doctor anything about your periods, then don't expect him/her to be able to give you much help.

If you are worried about hormones being out of balance, knowing the typical course of your periods is essential.
If you refuse to answer, you are only harming yourself and hamstringing the doctor from taking care of you.
If you cannot answer that, maybe you shouldn't waste everyone's time and cancel the appointment. If it means a lot to you to figure out your imbalance and acne issues, then you need to find some way of answering the questions.
If you dr visit is just for acne then they may just ask if it is worst during your period or ask when the first day of your last cycle was. There shouldn't be any more than that. For a hormone check they will do a blood test. The do not have to do a pelvic exam. If you don't want a pelvic exam you do not have to get one. If you are old enough to say no then NO ONE can force you! A parent cannot even make a doctor do it to you (legally). You could sue if they did.
i dont like them either asking the questions if they ask something like have you had sex or have you experiancing something abnormal just say no
