Of course something could be medically wrong with her, she needs to go see a doctor, or it might just be that she can't hold her pee, but I highly doubt that. She has urinary problems!
Unfortunately it's a common problem for some women as they get older. How many children does your aunt have? Is she a heavy woman? Both of those things can cause a bladder to become weakened. Basically, she needs to see her gyn doctor and have it checked out. There are non surgical things she can do to help or if needed there are surgical options. Only her doctor can tell which one she needs.

Yes it is a medical condition. It is called genuine stress incontinence.
Here, due to the age or previous operations, the pelvic muscles that support the bladder are lax. So, the urethra - the tube in which urine is passed - looses its support. This means that anytime the belly is contracted, and the pressure inside the belly increases , the normal closure of that tube is not possible. Thus, on laughing, sneezing or any such activities that pressurizes the belly, the bladder control is lost and urine is passed.
It is possible to control this medical and surgically. In primary stages, Keagle's exercise helps a lot. There are also some drugs available for that. The best alternative however, is the pelvic floor surgery. Here , the lax muscles are made taut again and thus the technical problem is solved.
I suggest she should see a gynecologist for the checkup.
