You can use any of the antifungal creams sold at the pharmacy to treat athletes foot. A medicated powder will also be useful to help dry the skin in the various folds, and yes, a piece of guaze or clean cloth is a good idea. Ideally you could get her into a bra to support the breasts and keep them up in proper position. Air and sunlight will also help a great deal-though I realize that is hard to do considering the spot. In the past, when I've had patients in this spot, we used a large ace wrap done in sort of a figure 8 to life the breasts up and hold them so there wasn't pressure on the rib cage from a bra, if the area were large and painful. The idea is just to find a way to lift the breasts up off the chest and abdomen so the skin can dry. Wash her skin gently with an antibacterial soap, then air dry it. Use a blow dryer set on the lowest level, and take care not to burn her skin, though. As a last resort, to get the breasts off the chest, you can also use paper tape sold by the first aid supply and simply tape things up and apart. It's not very comfortable, to say the least- but will give some relief from the rubbing. Once the skin can dry a bit, and you have the antifungal cream on it, the pain should lessen considerably. You may have to get a little creative, and it may not look very nice, and Grandma may feel a bit embarassed, but you really want to get things lifted for air and sunlight. And try to see if the doctor can't see her, or at least give you a prescription to treat it.
You need to buy some antifungal cream to apply there. Athletes foot cream, jock itch cream or vaginal yeast infection which ever cream is cheapest as they are all the same.

Go to your pharmacist, ask for an anti-fungal cream, apply this to the infection, and have her wash with an antibacterial soap. Definitely still take her to the doctor, though.
Any type of antifungal cream will help clear it up. Also, if you can keep something dry under the fold to collect any moisture, it will help to clear it up. You are very right in not putting cornstarch on it as it cause the yeast to grow at a faster rate. Try pushing her breast over a little to the side to help the area get some air. Maybe when she naps or is just resting, make sure she's not wearing a bra, and just push her breast over to the side a little. Any air is better then no air and will help it clear up sooner. Keeping the area clean and dry is the best thing you can do. I don't know about the butt cream it may hold moisture in. Wear disposable gloves when you treat her skin, use sterile gauze pads soaked with sterile saline solution (like baush and laum eye wash) on the area to clean it and pat dry. Cover after drying with an absorbing non stick pad. If the disposable wash cloths have perfume or soap in them it may irritate the raw skin. The doctor will give you a script for an anti-fungal medication and tell you how to apply it.
