The bacterial infection of your fallopian tubes can have many contributing factors. Yes, Tampon use can in some cases cause a beacterial infeaction and given that you have never been sexually active is probably the most logical cause.
Tampons hold moisture in a very damp and dark environment which is a prime breeding ground for bacteria which can travel to the fallopian tubes and uterus. FOr that reason - it is best to use them minimally and alternate with sanitary pads. Never leave them in for prolonged periods of time or overnight.
As far as polyps - while quite common they rarely become "big" trouble. However they do cause concern in that you will need to be vigilant about yearly checkups to make sure they dont grow back at the cervical os (opening) and cause issues with allowing bacteria etc to enter the uterus. Having them removed was prudent and in all cases does not mean reoccurence. This os one of those "wait and see" type areas that you will address as any issues arise.
Most likely the laparoscopy is the culprit for your continued symptoms. In that procedure air is actially introduced into the abdominal cavity to that structures can be readily viewed. Some of this air gets trapped at the end of the procedure and cquses that bloated stiff feeling in your abdomin ---it can take up to 10 days sometimes for that air to dissipate.
