The nails are to remove the cruddy stuff. Ya know, like the way you use a fingernail to dig out what's under the other fingernail.

Same rule applies to the sphincter. Dig and sniff.

The proctologist is wearing gloves, so I don't think it would pose an issue...unless they are like inches long! Just use your judgement, if they are dirty looking, even with gloves, that is unhygenic, and just walk out the door. Do get the exam done from someone else though, that is a lifesaving exam! If you are not comfy with the doc, shop elsewhere!
