How to get the form E112 ?

Any ideas on how to get the form E112, in order to get your treatments paid by NHS but having them done in another country ??

they have the forms at the post office and then they send you a card so you can qualify for treatment abroad
Your local Post Office will have this form for you.
How long will it take for my upper ear (cartilage) piercings to heal?
You can do it on-line too. I did and it cost nothing - takes 5 days to get the card, but I cant remember the link
