Should it be a concern if someone is working outside doing the lawn that they have trouble breathing?

Yes, my father has a bad heart and high blood pressure. He is not allowed to do the lawn because he could have a heart attack. This sounds like it could be the same with the person you are talking about. Bring them a glass of water and keep a close eye on them as they could be in danger of a heart attack or even a stroke.

Make sure that the medication being taken for any of these (if already diagnosed....) is appropriate/proper dosage.

Check the air quality index before going outside.
Never work outside from 11-4
Always have a drink of water (not coffee/tea/caffinated beverage) handiy, chilled but not COLD
Take frequent breaks in the shade
Never work outside alone or at least have someone check on you frequently (I have my son bring me a dring every 15 minutes to "check on me" and keep me hydrated.... I have asthma...)

And if it's a prolonged work session, or an on the job/can't get out of it typed of thing....they need to make sure that their pottasium level is checked regularly. Constant persperation/heat re-hydrating etc...can lower the person's potassium level. Potassium keeps the heart beating!! Heart beating properly circulates blood, absorgs oxegen....ties into breathing etc....(don't panic about this part unless it becomes really extreme, but do keep it in mind)
