Because the blood vessels, for whatever reason, can constrict ( a normal occurrence) that temporarily reduces the blood flow and bam, " headache". As you mentioned several factors that cause a headache, these factors can restrict some of the tiny vessels from blood flowing freely. This is a normal thing and we all get headaches. However, Migraine headaches are based on the same principals but, they are longer lasting and can be triggered by " external stimuli", even an allergy to certain things. I hope that this helps.

This will depend, if it is a miagraine headache the blood vessels in your head have expanded and allowed more blood than it should have, then the blood vessel will try to go back to size and this causes the miagraine.

It' s most likely the restricted cranial blood vessels. Sometimes it can be relieved by botox injections or the fungal preparation I can't recall the name of now.
