your insurance information should be on file with the clinic already. as for your social security card you should not have to bring it. In fact I have never had them even ask me for identification. As for moisturizers, try a facial moisturizer, there is a johnson and johnson product called Purpose, this is a really good product line and doesn't contain all that gunk that is bad for the skin.
