No, they are very accurate. If it's a hair follicle test your in trouble. Most people who ask your sort of question will fail for the appropriate reasons.

It is fairly rare. They would then do a retest or another test. They do once in a while make mistakes but it's not a good defense if you really did take the drug.

In my experience. it depends on the drug. If it is something that has to be inhaled, such as pot or crack, then there is the off chance that just being in close proximity and breathing the second hand smoke could be enough to trigger a positive result. There are also certain OTC drugs that will cause a positive result when being tested for cocain or heroin.

it is not possible to test positive to a drug you did not take with urinalysis. most drugs bye products are excreted via urine and this is the basis of the test conducted on those suspected to have ingested these drugs.
It is possible depending on what kind of UA you are taking. Alot of UA's that are taken for job's are the basic ones check for amphetamines, Marijuana and such. If you are on a strong medication for pain, your UA might show drugs. If you have been around when someone who is doing drugs i.e. marijuana and other inhalant drugs, you might inhale some and this can cause you to have a positive UA as well.
a urinalysis consists of a dip stick for white blood cells, red blood cells, a pH, a specific gravity, a nitrite to detect bacteria, a glucose, a ketone, a urine protein and a bilirubin. also a microscopic exam is done if it is needed. a urinalysis is not a UDS.. urine drug screen. the laboratory does not check for drugs when doing a urinalysis. that is a totally different order and different tests.
on a UDS a false positive is possible depending upon the kit used. poppy seeds give a false pos opiate. benedryl can give a false positive barb. lidocaine can give a false positive and some antidepressants have displayed a false positive for PCP. nothing is fool proof.
it is a total myth that you can display a false positive from being in the presence of THC smoke or crack smoke. there is no such thing as second hand positives on these substances.
I'm not an expert but i have a small medical background. i have never heard that it is a pr oven thing either way but to my knowledge the only way to get THC to be non self omitted is that you have to be in a room the size of a telephone booth in order to get what they call a contact buzz... as far as the comments that the people mentioned that poppy seed or poppy seed filled foods they where maintaining that you can test positive for weed (THC) or for opiates I'm not so sure that i would got unproved thoughts about that comment be cause the poppy plant that morphine,heiron,vicidon,or percs are derived from are the same poppy plant but from diffrent parts of the plant like the pistol is the part of the plant herion is derived from. and to the best of my knowledge the poppy seed that we cook with is in fact a diffrent plant all together. check on hightime.com.. good luck young grass smoker...lol!!
