a self certificate is a when you state you are sick yourself and has nothing to do with your doctor, most companies will accept this for up to 5 days.
If you get a sick note from your doctor, it will specify how much time they want to to be off for, i.e. 1 week, you would then be expected to go back to your doctor after one week for another sick note (if you still need one) or return to work accordingly. If they doctor suspects you might need longer and tells you this, it would be courtesy to inform your comapny that you might be off for longer.
Usually the only time you need to get a 'fit to return' notice is if you want to return to work before your sick note runs out.
For example, if you had an operation on your arm and the doctor gave you a sick note for 6 weeks and after 5 weeks you felt you could return to work, your company would not let you back as they could be in trouble under health and safety rules if you hurt yourself (they can also claim part of your wages back from the goverment and they would have to repay the extra week which is a pain). However if your doctor certified you were fit to return to work, then this superceeds the original sicknote and your company can accept you back with no problems on the health and safety side of things.
