you should have had a sick note 2 years ago, genuine people should not miss out on anything, go and see him asap.
You have to go to your GP first for him to examine you and agree that you are unable to work. he will advise you from there.

You can work, the goverment supports enough people who claim to have problems. Hell I have a bad back too and rh arthritis yet work rings around most people.

Yes you need to go to doctor ,but I would recommend you try Pilate's because I had Sciatica and lots of pain after a serious op and it has helped me big time it is a gentle start tell your instructor your problem ,and you will find it a great benefit,could really change your life back pains are horrible.
Due to the high volumes of untrustworthy and fraudulent people out there who ask for medical certificates in order to pay for their insuirance claims, your GP (if genuine) would not sign you unfit to work unless he/she absolutely thought it was necessary.
Call Jobcentre Plus at 0800 055 6688 (8am - 6pm Monday to Friday). They can get you started. And you can download all the necessary forms on their website. After you have talked to Jobcentre and printed out the forms, you just make a normal appointment with your GP, who should know what to do. :)
If your Gp thinks you are incapable of working he will sign you off for the appropriate period whilst refering you for the necessary treatment.
Assuming you have the correct National Insurance details, you can then go to the Social Security to see if you are entitled to welfare benefits.
