You would need to apply for SSI and wait for their decision on whether you get approved for SSI, if you do then you get Medicaid. I do not know about what state agency in Wisconsin would do that, here in Texas, its called DARS, Department of assisted rehabilitative services, they decide on Social security and SSI, in my case I filed for Social Security Disability, it took two years from the time I filed to get it, I had to get a lawyer. Call the department of social services in your state or town. I thought they had an obligation to help everyone. If your bad back prevents you from working, you might be able to get diability also and if you get diability you will get medicaid.
I'd say just fill it out and take a shot. Let them ask you questions accordingly because it seems like the rules bend according to certain people. Personally, medicaid has never really worked out good for me. There are a lot of loop holes you have to watch out for. But if you are determined, then do your best and then let them finish it accordingly.

To get medicaid is tough, you need to get medical records that prove you are truly disabled because with out proof you will be turned down faster than you will take to fill out the application. If you are disabled with your current problem then they will send your information and a disability form you have to fill out to a disability firm within the medicaid program. While they investigate your records they will want all the medical records from all the places you went to seek treatment but they will get them themselves by contacting the places you went to get seen. It takes about a month for them to decide if they consider you disabled or not it is a lot of wait time, but its tough to get qualified for assistance.
