I would recommend switching your records from one doctor to the other. It will help the new doctor see what you have been previously treated for, if anything, and will be much easier for you. New drs always want to know your previous medical history and this takes the pressure off of you to remember everything.
You will have to sign a release form, though; which you can do by going into your old office and filling out a release. This is due to all of the HIPAA laws. At that point you can either have the old office copy your records and give them directly to you or sign the release stating that you give them permission to fax all information to the new office. Also make sure that all information about your current meds is given to the new office. You do not want to be put in a position where you are prescribed something new that will interact poorly with any meds you are currently on.

Just call your preferred new physician and make an appointment. It would help your new physician greatly to have your medical records. Go to your old physicians office and request copies of your records or give them written instructions and permission to forward your records to your new physician.
