What should you get ruled out first: physical or psych problems?

My therapist says rule out physical problems first. A retired psychiatrist that I know says rule out physical problems first. Yet all the doctors I've worked with want psych problems ruled out first.

I'm confused about this, as I have to balance psych problems with severe physical challenges! And figure out medication interactions can be a feat meant for a giant.

Physical health issues should always be ruled out first as they can be the underlying problem manifesting itself as a mental issue and if so, once they are resolved there is no mental problem to deal with. Causes can be not only medications and/or medication reactions, but inbalances in the sytems of the body. (hormones, electrolytes, etc. that all work together to produce a normal state of being in the body). If you have a medical doctor that does not hold to the practice of ruling out physical problems first you might consider whether he/she is the best physician for you.

Honestly it's more of an opinion. One affects the other, so ruling out one will help narrow the possibilites of a diagnosis.

ah, the old "its not our problem, problem" This can be for a number of reasons. First, what is the problem? Since you didn't say, I can only guess here. Probably has something to do with pain or phantom pain.

The insurance may not want to pay for all the testing involved, so psychiatrists may want the otherside to take the loss and the otherside is basically doing the same. YOUR THE ONE who is going to have to decide who does the legwork by putting your foot down. First, before adding more pills, I'd do the physical side, to see if there were a physical reason for what was going on. Then, move on to the mental side.

If you rule out physical problems first, you can sometimes find the cause for the psychiatric problem. Thyroid being low can give depression as one example. If you find the physical problem, you can often take care of the others too.


As others have said here, you can consider either first. But the truth is, it's harder to "rule out" psych problems. So usually psychiatrists and other MDs first aim to rule-out conditions we can answer with a definite "yes" or "no". the physical problems. This narrows down the possibilities. hopefully to just one treatable diagnosis.
