First you need to do it in person. Apologizing over the phone is a big "No, No". You need to sit him down and have a real heart to heart conversation and let him know how much he means to you and how much you care or love him. You need to acknowlege that fact that you know how much you have hurt him. Finally you need to assure him that it will never happen again.
