How can I get my Medical History?

Since this question is in mental health I am assuming you mean your mental health record. By law, you are able to request a copy of your record from the agency you recieved services at. You just need to request it in writing and they have a certain amount of time to respond (I think 10 days). They can suggest that you get a summary of your record if they feel it might be harmful to you to see the actual record. But in most cases you can sit with a staff person and review the actual record and/or get a hard copy of the record. They may charge a copy fee.
By law, you are entitled to it (if you are an adult). You simply instruct your physician to provide you with copies. The same with hospital records.
