Since Zoloft and Lexapro are both SSRI's (Selective Serotonin reuptake inhibitors), it is unlikely, that he would be on both of them. Being on an SSRI and another type such as Wellbutrin is normal but only for certain periods of time. More likely, his levels of the SSRI were too high causing an imbalance of serotonin in his brain and then with the methodone wearing on his heart...
My answer would more likely be that they weren't both prescribed amounts.
