Aside from the philosophical debates on the matter- the difference between the schizophrenic and normal person has to do with how they perceive and interact with the environment around them. The majority of the world have a fairly common perception of the world around them, which defines and controls how we interact with our environment. In some schizophrenic patients there are detectable differences in the brain chemistry, but no one is certain what most of the differences actually mean in terms of brain function. In other patients, the differences are in the brain function itself- it just doesn't work quite right, for whatever reason. The schizophrenic patient does not function by the same rules as the rest of us, because they don't live in the same world. They accept as reality things the rest of us don't experience. That's why they hear voices, talk to plants, or sometimes think they can fly. In their world of functioning, they seem to be perfectly plausible possibilities. Unfortunately for them, the rules of our reality apply.
In some cases, medications will allow the control of symptoms and will allow the person to maintain a modicum of normal existence, and in some cases normal life. Most, however, do not respond to medications- which is why the schizophrenic patients remain the greatest challenge to treat, and why they constitute a majority of patients in long term mental health hospitals.
