Is it possible you have OCD (obsessive compulsive disorder)? Maybe check with a therapist. I think it kind of goes along with anxiety and panic disorder. I had symptoms as you described also and found out that I had this condition.

We've all been there. One day you'll realise no one is perfect and life isn't always fair. Just live your life one day at a time and remember that it's too short for you to worry about every little thing. "Stuff" is going to happen no matter what you do, or think. So stop already. Just try living for a day.
Try to relax and just be youre self, none of us are
perfect.
do worry about the things you say, stupid or not. Don't think so much, stress is a big killer, and a cause of depression, believe me. perhaps you should develop a more positive attitude and outlook in life. dwell on the positive, not the negative. you can start by smiling a lot. bask in the warmth of the love of your family and friends. and don't forget to count your blessings since there are many less fortunate people around compared to you. cheers!
I try to take the perspective that life is a merely some kind of demented game. That, in every day or situation that comes along there is a lesson that needs to be learned. There is a reson for it all. Life is about finding a balance. Decisons are optional and consequences are mandatory. Force yourself to not give into the impulsivity that seems to be hindering you. Train yourself to hesitate before making a decision. Try to see things from many different perspectives before allowing the situation to run away from the ability of your mind to control things. Outsmart your own self. Pause>Contemplate>Rationalze>T. speak and do.. This has been a focal point of my own self exploration and this is the method that works best for me. Just don't get so caught up in it all. Detach yourself from becoming OVER emotional. You have the ability to protect yourself from the harmful consequences of impulseivity. Hope this helps you :) good luck
1) Do your best to take care of yourself: eat well, sleep as much as you need it, get out and be with friends when you can (even if you don't feel like it!), keep up your hygiene and so on. If you take the time to take care of yourself your body will be more able to *help* your mind when your mind needs it.
2) Positive Mental Affirmations. Find some positive things online, copy them down, and get in the habit of saying them to yourself, out loud when you have quiet time. Affirm that you can only control yourself in life. Affirm that you are a good and worthy person at the end of the day. Affirm that people make mistakes all the time, and it doesn't make them bad people at all.
And so on. Reinforce your faith in yourself as a wholesome, worthwhile person, and realize that it isn't all your fault. You're only human and have limits just like everyone else..accept this and do what you *can*. Google that "Serenity Prayer" for a decent catch-all version of this.
3) Triage. Deliberately and willfully limit both the number of responsible things you have to be accountable for on any given day, and the number of things you will worry about messing up. Say to yourself when you get up, "I have items A, B, and C to be responsible for and do well today, and I only need to worry about things 1, 2, and 3. If all else fails I will ONLY focus on these things." Then write your list down and keep it with you so you stick to it.
4) Keep talking to people. Keep in touch with your friends and with other people..and let your concerns be heard. Once they are out of your head and in the realm of discussion, at least half the time they won't be as loaded with fear and worry as they once were. Get your fears out of the darkness inside your own head, and let your friends shine some light on them, let them help you *not sweat the small stuff* so.
Hope this helps you. :)
