does anyone know of any systemic questions for shift handovers?

it depends in what context the handover is being done,

if it was like a medical setting the questions i would ask would be
1. are there any new patients? if there are then get the info on them first.
2. any major changes to the existing patients
3.any staff who are off?

obviously you could ask a lot more questions than that but i think these would be the most important

just to remind this is in a medical context you'd have to change it to suit your own situation

If you're talking about hospitals/nursing homes then the nurse in charge should give you all the details you need! However if you felt concerned about someone, expect test results and don't hear anything said about your concerns, then ask your question before report moves on to the next patient.

Any concerns you have should be clearly written in your nursing notes, spoken about at the handover as well. I always would write my concerns (Patient's safety/acting according to NMC guidelines + Cover my back as well and, while giving handover report state that i felt concerned about such patient for such reason and add "Please observe" just as i have written in my notes too!

Having said that! Communication (Good communication) is essential so should you feel unsure about what someone said, relevant to your patients/shifts etc . And should you feel a little shy asking in front of other members of staff, tell your staff you need to have a word with the nurse in charge so they leave the duty room and start work then tell the nurse in charge that you would like to clarify what he/she said as you didn't hear too well.

Do not make the mistake of pretending everything's clear when it isn't ! Do not put your registration at risk!
