Answers:
Make sure you have your complete health record. Try to remember when this began or became a problem. Remember if there was some type of catalyst.
You will not have to ask for meds. Most psychiatrist start you on them to see itf they help, without any prodding. Most of them will make you sleepy and even more emotional for the first several weeks. Make sure the cure is better than the illness.

Most doctors start with, Why did you come to see me? And you should answer because I having uncontrolled mood swings and get very nervous around people. It is affecting my ability to ....

I would urge you to consider asking for a counseling referral if the doctor only discusses a pill as a remedy. Social anxiety is best "cured" with coping techniques as well as drugs. If it is just starting, a coping technique and a journal may help you overcome most of your problems.
Most drugs lose their effect and need to be bolstered by other drugs. It gets to be an unending cycle in which you can become depressed because of the medication routines.
Even without a referral I would like to suggest you look at a healthy diet and a walk. Just walk around the block. Then walk around another block tomorrow. Then walk around both blocks or to the park ... whatever, just increase your activity. It will raise your own serotonin levels without any drugs and you will not easily get addicted. Try gardenning, swimming, biking ...
Also, make sure you are getting all of your vitamin B's and Calcium, maganes, and potassium. Your thyroids, hypothyroids, and other control centers will not work without the proper balance of nutrients.
Try starting a journal so you know if anything is triggering or making your symptoms worse.
I wish happiness, normalcy, answers to life, came in a pill. But at this point I am not so sure. Pills do not increase your control, make you normal over-night, nor solve all that bothers you. They can help, but usually they also will trigger another problem, such as, headaches, being sleepy, uncontrollable crying jags, gain weight, lose weight, acid reflux, constipation, impotence, ... the list goes on and on.
You are entering the twilight zone, so make sure to map your journey.
you can say anything you like.
the doctor will probably have a few things to say, so perhaps just let him/her start the conversation.
