My brother spends more time thinking of how to study rather than actually doing it?

he just turned 21 years old and he is pre med. he is studying for his upcoming MCATS, and he is spending more time thinking of ways to study than actually just studying. i think he may have a problem. what should he do to just stay focused?

I suggest he forget about Med School where you study and spit out the material for tests and consider getting a PhD where he can THINK about things creatively.
His behavior is pretty common, and not really pathological. Studying is pretty daunting. He maybe can think of a self-reinforcing behavior-mod system for himself, to help keep himself motivated.

Otherwise, he's an adult. His behavior may be indicating he's ambivalent about medical school. Perhaps he really wants to do something else with his life! Let him work it out.
Your brother needs to create ways to study from flash cards and having someone test him to giving himself a schedule of studying such as 30 minutes and then 10 minute break. Also all distractions should be away because I remember finding new projects just to engage myself in when I had to study. Also have him change the scenary from the bedroom to the library, or a park where he can be by himself to study. No IPOD because that will just get you to start dreaming and negate your studies.
