Hi as a ptsd sufferer, 20 years in Fire/EMS, 4 years as a victim advocate dealing with sexual abuse...and my own therapy I would really like to discuss this in more detail. I've had good experiences with EMDR and great experiences using EMDR on victims of sexual abuse. Without knowlege of your past or treatment, or signs and symptoms highly suggest that all that trauma is still filed away...and has not been processed. The good knews is a host of symptoms can go away if you can do that/if you haven't done that..and I'm taling about things stored in a primitive part of the brain...one is not aware of. There are almost universal lies and feelings one believes in the subconscious that most abuse survivors have. There are tools to unlock and move those memories/ pictures to a different area of the brain that allows them to be processed.one is EMDR. Please email or IM if interested, hope I haven't bored you
My question would be... how long have you been taking them? If not too long then, you have to give it time to reach therapuetic levels in your brain, esp with the SSRI's they take about 4 to 6 weeks to fully take effect.
If it's been a long time, then talk to your psychiatrist and tell them it's simply not working and they will do something else.
If ever you feel like you are going to harm yourself... GO TO THE HOSPITAL!! NOW!
Tell your doctor. There are other medications, maybe something else will work better for you.

I have a mild form of PTSD. In my case there is one person who triggers it or aggravates it (because that person caused it) and luckily if I avoid this person then I'm fine.
But let me tell you that I spent a week with this person earlier this year and every night at bedtime like 10pm or so I went through agony and couldn't sleep, couldn't even get a sense of peace, couldn't watch TV, called everyone I knew (who would answer) and nothing helped. It is hard to describe. I just felt emotional in the extreme. I just felt deeply, deeply, deeply disturbed. This would last til 4am or so and I could finally drift off emotionally exhausted.
Also someone very close to me used to self harm so I feel I can relate to you somewhat. Email me if you want.
For a more brief specific answer to your question, you may want to consider telling your doctor that you want to try another SSRI. They are all different, and I went through several different ones as well as antipsychotics.
