Answers:
You are asking for help from us, but, truly the only people who can help you are doctors and psychologists. A doctor can prescribe you a medication to make you fall asleep and a psychologist can help with the images.

Sounds like you have upset your sleeping pattern by always falling asleep at 5am.

There is nothing "wrong" with you, but it sounds like that is what you're afraid it hearing. Plenty of people have trouble with sleeping for various reasons and its very simple to have it resolved with a visit to the doctor.

If you get your sleeping problems fixed you will feel more energised, be more organised for school and be happier.

Its hard not to turn the TV off but it's keeping you up so why don't you listen to music instead? Watching TV will make your mind go a mile a minute and you probably can't sleep because you're thinking too much. You can also sleep with a small light on if the darkness bothers you.
Try not eating past a certain time in the night (7pm maybe?) because sometimes digestive problems can keep you awake.
If you shower at night - have a hot shower right before you go to bed to relax your muscles.
If you're stressed, try organising the things you'll need for the next day before you go to bed eg. Pack your bag, have your work done, set an alarm.
Try wearing an eye mask if you can't stop opening your eyes and thinking about things.
Make sure your bed is confortable and your room well ventilated.
