You may suffer from demophobia (fear of crowds), so see page 9, at http://www.ezy-build.net. (.net.nz/~shaneris)
See a therapist. This may be from tourettes syndrome to claustrophobia. A therapist will be able to help you identify and resolve the issue.
sounds like an anxiety problem, something like xanax or valium may work for you, go see your doctor and he will make a decision based on your symptoms of what the best medicine would be for you...good luck, 3 depp breaths and exhale through your nose works good at times. try that as well, hope you get better, anxiety is a very difficult and trying problem to have.
It sounds like you just need to step outside the box. Breath! Try to block out all the noise and try to clear your head and imagine yourself in a field of tall grass, sun shining and your alone. Sometimes that happens to me when I'm really stressed out and my brain can't seem to take anymore and noise is the worse thing for me at that time. I doubt that you'll actually scream but try that and see if it works.
When I go to the library I feel like I need to do that. I don't know whether I want to do it just to see someone else's reaction or because it is just something youre not supposed to do in the library. The only thing I know of is to step outside and get some fresh air, or talk to someone while you're there to get your mind off of the silence.
I wouldn't worry about it being anything too serious; I get the same urge all the time...even in crowded public loud areas...I think it's just the urge to get a reaction out of people or something. I have to control myself from doing it and so do you...I'm sure a lot of people have urges to do stuff like that...just to see what people will do. I have been at church and have visually pictured myself jumping off the balcony...but I don't do it...just a random weird thought that I let pass on thru b/c I know it's crazy and stupid...start worrying when you actually start acting on your thoughts, that's a whole other story.
I have experienced this feeling many times myself. I find that redirecting my mind away from the people and just concentrating on the job helps. People may look at you a little strangely if you try something else I do. Whistle, hum or sing while you are working. This is really helpful in eliminating stress.

Anxiety disorder. ask your doctor about "celexa" to control the stress and anxiety of crowds. It could give your life back to to you.
