Every night I fall asleep and end up screaming and I'm still screaming when I wake up,what can I do?

First, I'll state the obvious (at least to me. I am a PhD Psychologist and MA - Social Work) First take care of yourself. Because we are caring people, we do care more deeply about people and their problems. That's OK! You don't necessarily have to quit caring or quit helping people.
You do need to strengthen your boundries. It can help to write or talk to a person who will keep confidences. This can help you "let go". Before going to bed you need to FEEL the divisions between what are your problems and what are other people's problems. Doing visualizations about this helps me.
If this does not stop your nightmares, or when you have nightmares in the future (Everybody does from time to time) get up. Do an activity. It just has to be "something". Watch an infomercial. Write a thank you note to someone who doesnt exect one i.e. the mail man, your favorite weather an at your local TV station. This helps change your focus from feeling bad because of the dream to wonderful feelings anticipating someone's surprise and inner-smile when they recieve your surprise thank you.
This changes your focus which is a lot in this life.
Then, what I do is - go back to sleep SOMEPLACE ELSE. The sofa, spare bedroom, once when I had no other bed to finish my sleeping on, I pulled the mattress off the bed and pulled it across the room.
These are just some tricks that work for me. Hang in there!
Stop screaming when you wake up would solve half the problem and stop being a gay wuss.
Talk to a trusted adult other than someone in your family. Get some counseling if possible.
See a good doctor. Maybe you have repressed memories.
Do you have nightmares? Helping others is nice, but you have to help yourself first. I think you really need someone to talk to. Venting helps heal the body and mind. Be with people who listens, or maybe see a therapist about your issues. You may have better dreams later on life.

If you are deprived of sleep every night you should see a professional because accumulated sleep loss can affect every aspect of your life. It will probably only take a short while for someone to identify what is upsetting you. Make an appointment and soon you'll be sleeping like a baby.
Sound-proof your walls, floor and ceiling for starters! Ok, seriously, you sound like you have some major unresolved issues which you're refusing to confront during your waking hours, and so they are fighting their way out into the open, forcing you to deal with them, during your sleep time.
