This question is not easily answered. The word insane is not used in clinical settings. While mental health professionals among themselves may use words like, "crazy or nuts", the word insane is not readily used. Most commonly, a person is psychotic when they are out of touch with reality. Commonly this means hallucinating. People who are psychotic manifest auditory hallucinations or visual hallucinations or both. They may also believe they are
receiving special messages from the radio, TV or their ipod. They can also believe they are getting messages from aliens or animals.
Unfortunately, sometimes psychotic people hear voices telling them to harm others or themselves. They may also manifest odd beliefs such as believing their family is not really their family.
The most pronounced problem with psychotic people is they do not believe they are psychotic. It is highly unusual for someone who is psychotic to realize the voices they hear are not real (after all, they are hearing them).
Because of this problem, such people may be paranoid, guarded. If a person is suddenly hearing voices inside their head and they are able to verbalize it then that person is not out of touch with reality. They know they have a problem for after all, hearing voices is not normal. Such a person is more than likely to readily accept help.
The field of mental health is very interesting. Psychiatry is not a precise science. There is so much we do not know. The good news is there is so much we do know that we did not know before.

Sane people always have a voice inside their head telling them right from wrong. Insane people hear voices coming from dogs, trees, heaven, etc.
