The question I want to ask is..what one of my problems from what I described could be causing me to have outbursts all the time.I have them quite often, and it's like someone else is controlling the wheel, most of the time I try really hard to fight it but it is impossible..also when I call her names or say mean things to her I don't think before I say them, anotherwards, it just comes out.like someone else said it..and no I don't have tourettes or I am not schizophrenic just to let everyone know. It is uncontrollable

Have you always had these problems or did they start up recently? It's very possible even the medicine you're taking is causing it. I've been trying the medication game for a few years and so many of them have given me more problems than I've ever wanted... (don't let that ruin your hope though, I'm now on good meds that have helped me so much and I'm very content!)
I have OCD, ADD, anxiety, and panic disorder (with some paranoia) and I feel I might relate to your question a bit (and wow I dated a bi-polar girl before).
Anxiety and OCD tie closely together and anything that triggers a bad mood can be backed up with the two full force, thus the out bursts. I don't know what it is exactly that you're "snapping" at your gf about but really try to keep yourself in check and pay attention to when it happens.
I know that I often completely snap at my boyfriend without thinking all the time and find myself apologizing because I didn't even want to say that...
It's very possible that her bi-polar could play a part in the problem too since it could cause her to be less patient with you. Ask her to help you, you've been together so long you should keep up the bond! Unless it really is too serious and you need some space.

how much do you like your mom and your gf? maybe just a "time out" period when you feel yourself getting up to that point would work? Or tell your mom and gf when it is about to happen and ask them to help you out of it? get them to talk to you calmly about how you feel... would you like to get that kind of treatment from them? have you talked to your psychiatrist and therapist about these bouts? what do they say to do?
