My sister in law has had to sign off on all 4 of her kids because she couldn't take care of them properly. They all had mental and/or physical problems. She hasn't been the same since, she doesn't take care of her self anymore and has mental issues because of it. But it is her fault cause she didn't go and get help for the kids. The state will help parents if they ask, or go and apply. But she didn't and let it get so bad that her oldest kid was 5 and didn't use the bathroom on her own yet.
The state comes in and does an inspection tells you what needs to change for you to keep your kids. Then they stop by unannonaced and if things are not different they take them away. You have to give up rights if they take them away, and have already tried to work with you.

My grandmother had to do that with my uncle. He lived in an institution all his life until he was about 46. She visited him frequently, but was not able to care for him at home.

Honestly, if a child is emotionally impaired, separating that child from his homelife would make it WORSE!! There are state programs that help with the costs of caring for special-needs children, if that is your concern.
I've had to take a lower-paying job to stay home with my special needs kids, so you're not alone!
Haven't had to do that, but have seen it done. The parent retains custody and can spend as much time as they want with their child, it is primarily a way to force the state to get involved because our health care system is so broken.
You don't lose your child or any rights just the health care cost responsibility, you might still have to pay something to the state for all other care.
