I think it depends on the laws in your state, but just about everything is confidential with the exception of a few things:
-a specific threat of suicide especially if you have a detailed plan
-a specific threat to hurt or harm someone else or homicide
-child abuse of any kind including physical, mental, sexual, neglect, etc
Typically if anyone is at risk (including yourself) then confidentiality in that case can be broken to ensure safety of everyone. In other cases, they may recommend treatment or hospitalization but typically can't do an involuntary commitment unless you are a threat to yourself or anyone else or have gotten to a point where you are unable to care for your own needs.

I'm pretty sure the answer to that question is no. I think everything you say is private. EDIT I think the word I'm looking for is confidential.
Ask them. They're obligated to tell you. If you say you want to injure yourself or others that's usually when everyone freaks out. I'm reading up on that in my counseling class. I'd try to get therapy instead. Ask them or others if there are any sliding scale clinics if you can't afford it. You may also need medication to stabilize you if you're feeling really bad.Trust me. They're not excited to do anything unless you say something like what I mentioned.
