First off, there's no "test" that conclusively proves adhd, and anyone who tells you otherwise is misleading you. You can read more about this in the mythbuster section at http://www.adhdtesting.org/ .
If you want a diagnosis, you can ask your doctor for one. If s/he doesn't give you the right diagnosis, of course, you can shop around for another doctor. I knew a woman who got a RitalIn script for her kid in a grocery store parking lot from a doctor she met in the checkout line.
In my opinion, a better question for you to ask would be, "How do i determine whether i have adhd or not, and what are my options to control it?"
ask your doctor for a referral to the right kind of doctor to talk to and give you a medication prescription.

everyone wants to treat it, and it is over diagnosed, so to get aproper diagnosis, first see a neurologist to rule out any central nervous system issues, and if all is normal, the nuerologist will either diagnose it and treat it or more likely send you to a pyschiatrist that works with a pyscologist, as you will need both to treat you properly.
talk to your physician about it. most clinics have computers with the software for the test (yeah its a computer test) after the test you take an evaluation and if u say you couldnt stand to sit in the small room and take the boring test u get diagnosed haha. its a little more complicated than that but thats basically how it goes
