I guess my question for you is, why hasn't he seen a doctor!?
Possible causes I could find on various medical sites: Gonorrhea, non- specific urethritis, chlamydia, trauma to the urinary system, infection in the sperm producing glands, cancer.
TAKE HIM TO A DOCTOR ASAP
If this is with ejaculation (hematospermia), it can be a benign condition without clear cause. It would be best to check it out with a urologist to rule out infection,etc.
