Of course there are many other reasons for testicular pain such as epididymitis or orchitis (infection/inflammation of the epididymis or testicle itself). However, these sources of testicular pain usually do not just arise with intercourse or ejaculation. You mention feeling something like a vein in the right scrotum. This may be a varicocele, which is essentially like a varicose vein in the srotum and usually found on the left side. Although there may be pain associated with this, it is not isolated to times when you reach orgasm.

I would recommend that you see your urologist who can evaluate you in person, perform the neccessary work-up, and make the appropriate recommendations. Wish you the best.
