Testicular pain may relate to a variety of causes such as trauma during intercourse with possible twisting of the testicle, referred pain from prostatitis (infection/inflammation of the prostate gland), or ejaculatory duct obstruction with pain due to possible blockage of the ejaculatory duct (perhaps on the right side) with inspissated (sludging) sperm causing pain when the semen and other secretions behind it cannot drain when you reach orgasm. Of course there are many other reasons for testicular pain such as epididymitis or orchitis (infection/inflammation of the epididymis or testicle itself). However, these sources of testicular pain usually do not just arise with intercourse or ejaculation. You mention feeling something like a vein in the right scrotum. This may be a varicocele, which is essentially like a varicose vein in the srotum and usually found on the left side. Although there may be pain associated with this, it is not isolated to times when you reach orgasm.
I would recommend that you see your urologist who can evaluate you in person, perform the neccessary work-up, and make the appropriate recommendations. Wish you the best.
