One should run, not walk to the phone to call a sex therapist and tell him/her what you've written here. I'm sure the therapist can only counsel you on one issue at a time, but at least you'll have the satisfaction you'll eventually be treated for all of them. And as you progress, one area of therapy may overlap into another area. I'm sure the therapist will learn you need to increase your self esteem in order to help alleviate some of the issues. Be very patient with yourself and if you have a current sex partner, tell him/her about your issues. If you are too afraid to talk about sensitive issues with him/her, the partner isn't going to help your issues at all. You must trust who you trust enough to have sex with, as in today's world, there's enough new STDs to encourage you to only sleep with those you trust and you must always practice safe sex.
