After a cast is removed and the leg is set free to moove the muscles which were at rest for almost a month and free and the sudden activity causes pain . I have had it and believe me its not nice.
..........pain on it is natural dont worry even if it gets knocked after the cast is removed.

go to the ER he could have refractured it or may have sprained it. or it could be just stressed from the trauma after being in the same position for so long but better safe than sorry.
