The main connotation of that phrase has always been, as long as I have lived, to lose your virginity or to be the cause of a young woman losing hers. If you have sex-ed classes you will know that women have a membrane of some sort called a hymen in their vagina. When they have sex for the first time this is torn/ruptured and will bleed. Honestly, I'm not so sure, either all the girls I slept with in highschool were either not virgins or that doesn't really happen. The more active and athletic a girl is supposedly the less likely that is to happen.
That is the main use of that expression. However, it can be used in reference to you "losing your virginity," so to speak, in anything. It can simply mean you successfully completed your first try in anything other than sex. Although, I personally wouldn't overuse that expression, it will make you sound like a moron.
