lung transplants? Any info?

Question: My father is 61, and has Emphysema, he isn't on oxygen, his heart is fine, he has use of 1 lung, but, he passes out, or is so winded and fatigued, he can barely do much.
He uses a neubulizer, and, has a "breathing machine" he calls it, he takes treatments throughout the day and night.
He is seeing his family Dr, she is a small town Dr., any suggestions on how to go about getting on a list for a lung transplant?

I have had several kidney transplants.......I've heard that Pittsburgh is the place for lung transplants.
