My hubby who is 58 just had a radical prostatectomy but in Canada. He had it done laporascopically. His PSA was 4.2, and when we went for his one week check-up, they said they found the prostate had far more cancer than they thought. He was thinking of doing the watchful waiting because his PSA's were so low, but in hindsight, he did the right thing. He outright refused the radiation for several reasons. One, it can burn the tissues surrounding it, including the bowels, and also, if the radiation does not work, you are no longer a candidate for surgery.
He breezed through the surgery, it was amazingly easy for him, but somehow he picked up the flu and the first week at home he had to deal with that as well! But he is on the mend now, has minimal scarring and when they removed the catheter, he had very good control of his bladder. He gets up every 3 hrs to go to the bathroom, but has very little problem with dribbling.
The surgeon is very sure he managed to save the nerves for both incontinence as well as erectile disfunction.
We are convinced he made the right decision because of the level of cancer they found when they did the post-surgery biopsy.

One of my best friends had a radical prostatectomy 3 1/2 yrs ago at the age of 50 done by a famous surgeon here in Texas. Since then he has had a PSA of 0 with every test, but has been impotent and dribbles constantly and has to wear underwear liners. Not all surgeries are successful at preserving urological and sexual functions. BTW he IS glad he had the surgery done. The cancer was an aggressive kind and was right to the edge of the prostate gland, but not beyond it. The Drs. told him that in another 6 months the cancer would have spread and he'd have been terminal. He would rather be alive and impaired than the alternative.
